The Management of Traumatic Transection of the Pancreas Four Cases Report and Review of Literatures


Abstract
The management of traumatic transection of the pancreas is a perplexing problem to surgeons.
Because the early diagnosis of the pancreatic injury is a difficult one in a patient who had received
traumatic injury on the abdomen, the early treatment is delayed and it results in many complications
and increase the mortality rate.
Since 1856, Jaun was reported the first instance of complete repture of pancreas, many reports of
pancreatic injuries were presented but the surgical management was differed by authorse Here, we
report four patients with traumatic transection of the pancreas as a result of abdominal trauma without
penetration. The four patients were injured pancreas itself without injury to other organs.
The managements of this serious problems consists of immediate definitive operation with control of
hemorrhage, remorrhage, removal of the severely damaged pancreas with splectomy, Roux-en-Y proximal
pancreato-jejunostomy with biliary decompression aed Roux-en-Y distal pancreato-jejunostomy.
The result of surgical treatment in our four surviving patients were generally excellent except the
complication of pancreatic abscess and fistula.
